OFFICE HOURS
WEEKDAYS
08:00AM - 05:00PM
TO SCHEDULE AN APPOINTMENT
Please call (731) 587-5138 and press 1 to schedule an appointment. Appointments can be made at any time during regular office hours.
We realize that your time is important — and so is ours. Please provide 24 hours notice for cancellations.
To manage our schedule fairly, we give priority to patients who have an appointment and arrive on time. Please try to be prompt for your appointment time. You must bring your insurance ID card to your visit as well as your co-pay.
WALK INS
Patients who arrive without an appointment may be squeezed into the schedule; however, walk-ins are likely to experience a long wait or be asked to schedule an appointment in the next available opening.
LATE ARRIVALS
Patients who arrive more than 15 minutes late may be asked to reschedule or may need to wait until they can be seen.
NO SHOWS
Patients who have two or more “no shows” per year will be considered for possible discharge. Patients are counted as “no show” if they don’t cancel at least two hours prior to their scheduled appointment.
FORMS
SCHOOL, CAMPS, & SPORTS
Your child will require many forms to be filled out by our office from year to year. When your child’s school, camp, or sports program requires a healthcare provider signature on a form, please provide it to us with as much information as possible filled out. Please provide forms to our office AT LEAST TWO WEEKS PRIOR to the time you need them to be returned.
WHAT TO BRING TO YOUR FIRST VISIT
Martin Children’s welcomes new patients. Choosing the right pediatric practice for your family is a big decision, and we thank you for considering Martin Children’s Clinic. While we realize you have to consider location, personalities and your own family’s individual needs, we can assure you that we do our very best to offer the finest pediatric care possible to our patients and families. We offer live (and friendly!) operators; knowledgeable nurses who quickly return calls; a beautiful, warm office; and of course, wonderful, dedicated practitioners.
To help expedite the first appointment, we ask that you arrive at least 20 minutes early and please bring:
- Current prescriptions
- All insurance information including cards. Keep in mind that it is important for you to ensure that we have ALL of your insurance policies on file. We only have a limited number of days to file claims on your behalf. Failure to provide us with Primary and Secondary policies will result in you being responsible for the total balance of your visit. This includes Tenncare as they will not cover your visit if you have a primary insurance that has not been filed.
- Immunization Records
AFTER-HOURS CALLS
In case of an emergency that cannot wait for a phone call, call 911 or proceed directly to your nearest Emergency Room.
Established patients that need to talk to a provider after hours may call 731 819-9084 to leave a message and await a return call. With some problems we may be able to help you over the phone or direct you where to go for care. It is also very helpful to have any medication your child is taking and your pharmacy telephone number when the provider calls back.
TELEPHONE INQUIRIES
A phone nurse is available to answer questions during normal business hours. If you leave a message she will return your call as soon as possible.
We ask that you not use this to request an appointment; instead you should press 1 to speak to the receptionist.
MEDICATION REFILLS
You may leave a message on our nurse line to request a refill. Please do not leave refill requests on the weekends as the message will not be checked until the next business day. We require 48 business hours to process your refill request. Prescriptions may be picked up at the front desk. Antibiotics will not be prescribed without the child first being seen. It may be deemed necessary to see your child prior to refilling some medications, specifically if your child has missed their most recent ADD recheck their medication will not be refilled without an appointment.
